The Chapter Needs Assessment form (due to Headquarters October 15) will be available on the International Web Site August 1. This form, referred to as the CNA, is required for a chapter pearl. The form must be filed electronically. Click the link below to submit the form online. The link will take you to the International website:
Follow these steps:
- Click on Members Only. You will need to type in your username which is your last name with a capital letter followed by your membership number. Your number is on the top left corner of the mailing label on the Kappan.
- Your password is ADK2016 unless you have changed it. If that does not work, click on Forgot password? and follow the instructions.
- Go to Resource Library.
- Go to Chapter Officers.
- Look under Chapter Documents and Forms. Scroll down until you see CNA Chapter Needs Assessment Form.
- You will also find a sample of the form in the same column.
Upon submission of this form you will receive a confirmation email to the address you provide. You will be able to print a copy for your chapter files from the confirmation email.
Your report will also be automatically forwarded on to your S/P/N Membership Consultant (via the Regional Membership Consultant).
